Important Reminder for parents seeking Medical advice. Most insurance carriers provide Nurse Triage hotline services as part of your benefits. Call your insurance company for further information.
Office Hours: Regular hours
for scheduled well child and sick child visits are from 8am to
5pm Monday through Friday with some of our physicians offering
7:30am appointments. Calls to schedule appointments can be made
from 7:00am to 5:00pm Monday through Friday. Our office
does not close during the lunch hour. We are happy to be
able to offer Saturday hours for urgent care. Visits are
by appointment only at this time. Saturday hours will vary
by location. Please call 480-609-8100 for same day sick
visits.

What to bring to your appointment: You must bring a valid insurance card and your co-payment (if applicable) to each appointment. Also, please try to bring your child's immunization record to any well check appointment. If you have new insurance and you have not yet received your insurance card, you will need the following information: insurance company name and phone number; claim address; co-payment amount; and policy holder name/date of birth/phone nubmer, policy number, and suffix.
Well Checks: Please schedule well check appointments as early as possible to help ensure the availability of your Primary Care Physician. July-September is a very busy time for well checks as everyone is preparing for the upcoming school year. Please bring your child's immunization record to all well check appointments.
No Shows and Late Shows:
If you will not be able to keep an appointment, please call
our office to cancel as far in advance as possible so that we
can offer that appointment time to someone else. As a courtesy,
we will give you a reminder call prior to your appointment. For
your convenience, we have a 24 hour cancellation line: 480-609-8102. If
you are more than 15 minutes late for an appointment, we may
have to reschedule you to a future date.
Pharmacy Refills: Please call your pharmacy to expedite all refills excluding controlled substances. For controlled substances, leave a message on the medical assistant's voicemail which you can access by calling the main phone number: 480-609-8100.
Message Results Line: The patient message line allows you to retrieve messages from your doctor regarding lab results, diagnostic results and referrals. You will be notified by phone once your message has been entered by your doctor and is ready to be retrieved, usually in 48 to 72 hours. If you are waiting for a referral, most referrals take 5 to 7 business days to process through your insurance.
